This Privacy Policy applies to the public website and marketing interactions. For protected health information (PHI) handled on behalf of a covered entity client, our HIPAA Notice (below) and the applicable Business Associate Agreement control.

  • Cookies & similar technologies: we use essential cookies to keep the site working and limited analytics cookies to measure reach. You can disable cookies in your browser; some site features may not function without them.

P.2 How We Use Your Information

  • Respond to your audit, strategy-call, or contact requests.
  • Deliver the Blueprint and fulfill service agreements.
  • Send purchase confirmations, receipts, and service communications.
  • Send marketing communications about Sy I G services you may find relevant (you may opt out at any time).

P.4 How We Share Information

Sy I G does not sell your personal information. We share information only as needed to operate:

  • Service providers & vendors that help us run the business (payment processing, hosting, email, analytics, CRM). Vendors that may access or process PHI are bound by Business Associate Agreements (BAAs) as required by HIPAA.
  • Legal and safety — where required by law, court order, or to protect rights, safety, or property.
  • Business transfers — in the event of a merger, acquisition, or sale of assets, information may transfer to the successor, subject to this Privacy Policy.

P.5 Data Security

We use reasonable administrative, technical, and physical safeguards appropriate to the sensitivity of the information, including encryption in transit (TLS/HTTPS), encryption at rest on cloud platforms we rely on, multi-factor authentication on internal accounts, role-based access, and activity logging. For ePHI, the HIPAA Security Rule technical safeguards of 45 CFR §164.312 apply (see HIPAA Notice). No system can be guaranteed perfectly secure; we continually review and improve our controls.

P.6 Data Retention

We retain information only as long as needed for the purposes described above, to comply with legal, tax, and contractual obligations, or to resolve disputes. Records relating to PHI are retained for at least six (6) years from creation or last effective date, as required by HIPAA.

P.7 Your Choices & Rights

It is not directed to children under 13, and we do not knowingly collect personal information from children under 13. If you believe a child provided information, contact us and we will delete it.

P.9 Testimonials, Case Examples & Results Shown on This Site

Testimonial & Results Disclosure. Testimonials, reviews, quoted figures, star ratings, revenue examples, percentage gains, and case studies displayed on syigmed.ai and related Sy I G marketing materials are presented as industry-standard illustrative examples representative of outcomes achievable within the AI-powered healthcare marketing category. They are provided for educational and illustrative purposes only. They are not a promise, guarantee, warranty, projection, or prediction of the specific results any individual practice will obtain from engaging Sy I G. Individual outcomes vary based on practice specialty, market, competitive landscape, baseline reputation, patient volume, staff implementation, compliance posture, and many other factors outside Sy I G's control. Any first names, last names, titles, photographs, or practice descriptors appearing alongside testimonial text are used to illustrate the profile of a typical healthcare client and should not be read as a claim that the specific named individual is a current client of Sy I G. Sy I G believes, based on the capability of its AI systems and methodology, that it can produce results meeting or exceeding these industry-standard examples, but no such outcome is guaranteed. Before purchasing any service, you should conduct your own diligence and, where appropriate, request a written proposal and measurable performance terms.

🔒 HIPAA Notice · 45 CFR Parts 160 & 164

HIPAA Notice

Sy I G's role, obligations, and safeguards under HIPAA

H.1 Sy I G's HIPAA Role

Synergy Innovate Group LLC ("Sy I G") is a HIPAA Business Associate to its healthcare clients (each a "Covered Entity") for any engagement that involves Sy I G creating, receiving, maintaining, or transmitting Protected Health Information ("PHI"), including electronic PHI ("ePHI"), on behalf of that Covered Entity. Sy I G is not itself a healthcare provider, health plan, or healthcare clearinghouse, and syigmed.ai is a public marketing site that does not collect PHI from the general public.

H.2 When HIPAA Applies to Sy I G

HIPAA obligations between Sy I G and a Covered Entity are triggered by a written Business Associate Agreement ("BAA") executed before Sy I G accesses or handles PHI. Examples of Sy I G services that may require a BAA include HIPAA-compliant chatbots, AI voice agents, patient reactivation campaigns, reputation/review workflows that reference patient encounters, and any workflow involving patient-identifiable data.

H.3 Permitted Uses & Disclosures of PHI

Sy I G uses or discloses PHI only (a) as permitted or required by the applicable BAA and HIPAA, (b) to perform the services described in the BAA or service agreement, (c) for proper management and administration of Sy I G, (d) to carry out Sy I G's legal responsibilities, or (e) as otherwise required by law. Sy I G does not sell PHI and does not use or disclose PHI for marketing purposes without the Covered Entity's written instruction and any required individual authorization.

H.4 Administrative Safeguards — 45 CFR §164.308

Sy I G maintains a written Security Management Process including an annually reviewed Risk Analysis and Risk Management Plan, a Workforce Sanction Policy, an Information System Activity Review program, Workforce Security and Access Management controls, HIPAA Security Awareness Training, a Security Incident Response program, a Contingency Plan, and periodic evaluations. Formal policies are maintained on file and made available to Covered Entity clients on request.

H.5 Physical Safeguards — 45 CFR §164.310

Sy I G maintains facility access controls for the home office environment, workstation-use and workstation-security controls, device and media controls covering disposal, media re-use, accountability, and data backup and storage.

H.6 Technical Safeguards — 45 CFR §164.312

  • Access control: unique user IDs, emergency access procedures, automatic session logoff, and AES-256 encryption on workstations, mobile devices, and removable media.
  • Audit controls: logging of login/logout, access to records, configuration changes, privilege escalation, and data exports on systems that touch ePHI.
  • Integrity: mechanisms to detect improper alteration or destruction of ePHI.
  • Authentication: multi-factor authentication required on all accounts that access ePHI-related systems.
  • Transmission security: TLS encryption in transit and encrypted channels for any transmission of PHI.

H.7 Breach Notification — 45 CFR §§164.400–414 and HITECH §13402

If Sy I G discovers a Breach of Unsecured PHI, Sy I G will (a) immediately contain and investigate the incident, (b) conduct a four-factor Breach Risk Assessment per 45 CFR §164.402, and (c) notify the affected Covered Entity without unreasonable delay and no later than thirty (30) calendar days after discovery. The notice will include, to the extent known, a description of what happened, the types of PHI involved, the individuals involved (or approximate number), steps those individuals can take to protect themselves, what Sy I G is doing to mitigate and prevent recurrence, and a point of contact. The Covered Entity remains responsible for notifications to individuals, HHS, and media, and Sy I G will cooperate fully.

H.8 Contingency & Availability

Sy I G maintains a written Contingency Plan covering Data Backup, Disaster Recovery, Emergency Mode Operations, Testing and Revision, and Applications and Data Criticality Analysis. The target Recovery Time Objective for Tier 1 systems is 72 hours; the Recovery Point Objective is 24 hours.

H.9 Subcontractors

Sy I G requires any subcontractor that creates, receives, maintains, or transmits PHI on its behalf to enter into a written agreement imposing HIPAA obligations substantially equivalent to those Sy I G owes to the Covered Entity.

H.10 Individual Rights

Individual rights over PHI (access, amendment, accounting of disclosures, restriction requests, confidential communications, and right to receive breach notifications) are administered by the Covered Entity pursuant to that entity's own Notice of Privacy Practices. If you are an individual seeking to exercise rights in PHI held about you, contact your provider (the Covered Entity) directly. Sy I G will support Covered Entities in fulfilling these rights as required by the BAA.

H.11 Complaints

You may file a complaint with your provider (the Covered Entity) or with the U.S. Department of Health and Human Services, Office for Civil Rights ("HHS OCR") at hhs.gov/ocr. Sy I G will not retaliate against any person for filing a good-faith HIPAA complaint.

H.12 Disclaimer — Website Visitors

Do not submit Protected Health Information (PHI) through syigmed.ai public forms, the chatbot, or any unsecured email. Public forms on this website are designed for business intake from healthcare practice owners/administrators, not for patient health information. If you need to transmit PHI to Sy I G, request a secure channel and, where applicable, an executed Business Associate Agreement before sending.
